In a counterintuitive medical phenomenon that often confuses patients, gastroenterologists are warning that frequent loose motions can sometimes indicate severe constipation rather than typical diarrhoea. This condition, known as overflow diarrhoea or paradoxical diarrhoea, occurs when hardened stool blocks the colon, allowing only liquid waste to pass around the obstruction.

Dr Salhab, a practising gastroenterologist, explains that many patients dismiss this symptom, assuming they have a stomach infection or food poisoning. However, the underlying impacted stool can lead to serious complications if left untreated, including bowel perforation, faecal impaction, and severe abdominal distension. The condition is particularly common among elderly patients, those with chronic constipation, and individuals who regularly suppress the urge to defecate.

In Bihar and across India, where dietary fibre intake remains insufficient and access to clean toilets still poses challenges in rural areas, such gastrointestinal issues are increasingly being reported. Health experts recommend seeking immediate medical attention if watery stools are accompanied by abdominal pain, bloating, or a feeling of incomplete evacuation.

Treatment typically involves careful manual disimpaction, laxatives, and enemas under medical supervision. Doctors emphasise that self-medication can worsen the condition. Prevention focuses on adequate hydration, fibre-rich diets including seasonal vegetables and whole grains, and establishing regular bowel habits.
